When you approach the portal, go to the Portal Edit screen, click Scan Portal. Walk slowly around the portal to record. You can then upload the scan or wait to upload it later (preferred if you have limited cellular data). Note that the best portals are ones of significant interest, like statues, that can be fully walked around.
